You walk out of a ward round, OSCE station, or simulation lab with that familiar knot in your stomach. You think you did okay. Maybe. Then comes the feedback. A few sentences. Sometimes vague. Sometimes painfully blunt. And occasionally - rarely - it’s the kind that sticks with you for years.
Why is that?
It turns out feedback isn’t just a courtesy or a teaching tradition. It’s a neurocognitive tool. A powerful one. When done well, feedback shapes clinical reasoning, motor skills, communication, confidence - everything that matters in medicine. When done poorly, it can stall progress or, worse, train the wrong habits.

Feedback Is Not Advice (And That’s the First Mistake)
Here’s the uncomfortable truth: Most feedback in medical training isn’t actually feedback.
It’s advice. Or judgement. Or a checklist read aloud.

Learners who feel judged, rushed, or embarrassed often defend rather than adapt. Research from healthcare education consistently shows that psychological safety - the sense that mistakes are part of learning - is essential for feedback to work.
This doesn’t mean sugar-coating. It means clarity without cruelty.
The best educators say things like:
“Here’s what worked.”
“Here’s what didn’t - and why.”
“Here’s exactly what to try next time.”
No guessing. No mind games.
Feedback in Communication Skills: Where It Matters Most
In clinical communication - breaking bad news, informed consent, cultural safety - learners often think they performed well because the conversation “felt okay.”
But patients don’t grade on vibes.
Research-backed feedback frameworks (used in OSCE training worldwide) focus on:
Language clarity
Empathy markers
Structure and signposting
Active listening behaviours
Without external feedback, these skills don’t reliably self-correct. That’s one reason virtual patients and simulated OSCE platforms are gaining traction - they allow repeat practice with consistent, structured feedback.
And consistency matters more than charisma.
Why One-Off Feedback Fails
Here’s another hard truth.
Feedback works best when it’s iterative.
Single feedback events - no matter how brilliant - rarely change long-term behaviour. Learning accelerates when feedback is:
Repeated
Pattern-focused
Linked to future attempts
This is why modern medical training is moving toward longitudinal feedback models, not just end-of-rotation summaries.
Practice. Feedback. Retry. Adjust. Repeat.
That’s the loop.
What This Means for OSCE Preparation (Practically Speaking)
If you’re preparing for OSCEs - or teaching them - this research points to a few non-negotiables:
Feedback must be specific, not general
It should target observable behaviours
It must be close to the performance
Learners need a chance to apply it again
Reading model answers alone won’t do this. Neither will vague comments like “work on your communication.”
Structured feedback changes outcomes. Full stop.

Medical training doesn’t need louder feedback, harsher feedback, or longer feedback. It needs smarter feedback - designed with how humans actually learn, not how we assume they should.
